This Parental Consent, Assumption of Risk & Release (the “Agreement”) is made by the undersigned parent or legal guardian (“Parent/Guardian”) on behalf of the minor child named below (“Child”) and by Slime and Shine Studio LLC (the “Company”). By signing this Agreement, Parent/Guardian grants consent for Child’s participation in activities and agrees to the following terms:
1. Child’s Information & Participation
I, the Parent/Guardian, hereby authorize the Child to participate in all activities, events, and services offered by the Company. Participation is voluntary, and I affirm that Child is physically, mentally, and emotionally capable of participating in such activities.
2. Assumption of Risk
I understand and acknowledge that participation in slime-making and related play activities involves inherent risks, including but not limited to: slipping, falling, stains, allergic reaction, minor cuts or abrasions, and other injuries. I assume full responsibility for these risks, whether known or unknown, foreseen or unforeseen, arising from Child’s participation, even if caused in part by negligence of the Company or its representatives.
3. Release and Hold Harmless
To the maximum extent permitted by law, I hereby release, waive, discharge and hold harmless the Company, its officers, employees, agents, volunteers, and representatives (“Released Parties”) from any and all liability, claims, demands, actions, or causes of action, whether known or unknown, arising out of or related to any injury, damage, or loss (including personal injury or property damage) that may occur due to Child’s participation, except when caused by gross negligence or willful misconduct of the Released Parties.
4. Health Information & Emergency Medical Authorization
I certify that I have disclosed to the Company any known medical conditions, allergies, or special needs of the Child. In the event of injury or medical emergency, I authorize the Company and its staff to administer first aid and seek medical care on behalf of Child and transport Child to medical facilities if necessary. I consent to any medical treatment deemed necessary and agree to be responsible for all related costs.
5. Photo/Media Release (Optional)
I grant permission for the Company to use photographs, video, or other media that include images or likenesses of Child taken during activities, for promotional, marketing, or advertising purposes. I understand there will be no compensation for such use.
6. Governing Law & Severability
This Agreement shall be governed by and construed in accordance with the laws of the State of New Jersey, without regard to its conflict of law principles. If any provision of this Agreement is held to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
